Robotic versus laparoscopic surgery for middle and low rectal cancer: Robotic surgery resulted in significantly lower 3-year locoregional recurrence rate and better functional outcomes in patients with middle or low rectal cancer compared to conventional laparoscopic surgery in a multicenter randomized controlled trial (REAL) from China involving 1240 patients.

Air pollution impacts metabolic pathways linked to metabolic dysfunction-associated steatotic liver disease (MASLD), highlighting environmental contributions to liver disease: The authors used air pollution data combined with metabolic analysis from the UK Biobank to investigate the impact of pollution on liver disease. They identified metabolic changes in around 280,000 participants that were linked to air pollution exposure and demonstrated a potentially causal link between these changes and the development of MASLD. This research provides strong evidence of how air pollution influences metabolic pathways leading to liver disease.

Potential benefits of early antiviral treatment with tenofovir alafenamide (TAF) in chronic hepatitis B patients with moderate to high viremia: The ATTENTION trial investigates early treatment with TAF in chronic hepatitis B patients with moderate to high viraemia without severe liver disease to prevent liver-related endpoints. Participants were randomized to receive either TAF or a placebo. Interim results after a median follow-up of 17.7 months indicate that early TAF treatment reduces risks of severe hepatic complications. While this study is ongoing, the results may support early treatment initiation for hepatitis B patients.

Etrasimod as a treatment for eosinophilic oesophagitis: The sphingosine 1-phosphate modulator etrasimod resulted in a significant reduction of intraepithelial eosinophil counts and endoscopic improvements over 52 weeks in a randomized placebo-controlled phase II trial (VOYAGE). Symptom improvement was also evident in patients without a history of prior endoscopic dilatation therapy.
Bokemeyer B et al, J Crohns Colitis. 2025;19(4):jjaf051Comparative real-world effectiveness (RWE) of ustekinumab vs. anti-TNF in Crohn's disease: In this 12-month RWE comparison, overall clinical remission rates were similar between ustekinumab and anti-TNF. However, among week-16 responders, remission rates were significantly higher with ustekinumab. In addition, ustekinumab was associated with a significantly greater improvement in quality of life compared with anti-TNF.
