Risk of metachronous neoplasia after polypectomy in patients < 50 years of age: Patients < 50 years of age undergoing polypectomy during a screening colonoscopy had a comparable risk of metachronous polyps with high-grade neoplasia as compared to older patients > 50 years of age in this prospective cohort study. Therefore, identical follow-up strategies can be used in both age groups.

Time-restricted eating instead of calorie counting: New perspectives for the treatment of MASLD: A recent study from Korea shows that time-restricted eating (TRE) reduces fatty liver and body weight in patients with MASLD almost as effectively as traditional calorie restriction. The timing of the meals during TRE appears to be less relevant but rather the overall reduction in energy intake. Nevertheless, TRE could represent a practical and well-tolerated alternative for patients who have difficulty with continuous calorie counting.

Novel therapeutic approach in primary sclerosing cholangitis: This study identifies the tight junction protein claudin-1 as a key mediator in the pathogenesis of primary sclerosing cholangitis (PSC) and demonstrates an association with disease activity and prognosis. In preclinical models, claudin-1 blockade led to a significant reduction in inflammation, cholestasis, and fibrosis. Thus, this study opens a promising therapeutic perspective for a disease for which there are currently no effective drug treatment options.

The amylin receptor agonist eloralintid – A new approach to the pharmacological treatment of obesity: In a placebo-controlled phase 2 study in patients with obesity, once-weekly subcutaneous administration of eloralintid over a period of 48 weeks resulted in weight loss of up to approx. 20%.

Vedolizumab in early vs. late Crohn's disease (LOVE-CD): In this phase 4 study, treatment with vedolizumab was significantly more effective and had fewer side effects in patients with early Crohn's disease (disease duration < 2 years, naïve to advanced therapies) than in patients with longer disease duration (> 2 years, pretreated with immunomodulators and TNF antibodies). Clinical and endoscopic remission after 26 and 52 weeks was achieved in 31.4% vs. 8.6%.

New standard neoadjuvant therapy of pancreatic cancer (CASSANDRA): In a randomized phase 3 trial on preoperative therapy of pancreatic carcinoma, treatment according to the PAXG protocol (capecitabine, cisplatin, nab-paclitaxel, and gemcitabine) resulted in a significantly longer event-free survival as compared to treatment with mFOLFIRINOX (fluorouracil, leucovorin, irinotecan, and oxaliplatin) and could therefore be considered a new standard option.
