Tumor seeding during colonoscopy and metachronous colorectal cancer: In an analysis of primary and secondary tumors from patients with metachronous colorectal cancer the authors found that primary tumor cells might be seeded in a new location after biopsy of the primary tumor by colonization of the working channel with viable tumor cells. However, the risk of iatrogenic tumor seeding appears to be low (0,3–0,6%).

Hepatitis C: Real-world data confirm that the combination of sofosbuvir, velpatasvir and voxilaprevir is an effective and safe rescue therapy for patients with prior failure of direct-acting antivirals (DAAs). Resistance-associated substitutions do not have a relevant impact. Patients with liver cirrhosis and genotype 3, however, remain the most-difficult-to-treat patient group.

Non-alcoholic fatty liver disease (NAFLD): A current phase 2 trial demonstrates that norursodeoxycholic acid (norUDCA, 1500 mg daily) results in a significant reduction of serum alanine aminotransferase within 12 weeks of treatment. NorUDCA was safe and well tolerated.

Liver cirrhosis: A current study identified the hepatic venous pressure gradient (HVPG) as a good predictor of morbidity and mortality of extrahepatic surgery in patients with liver cirrhosis. HVPG values > 16 mmHg, especially ≥ 20 mmHg, were associated with a high risk of post-surgical mortality. With an HVPG ≥ 20 mmHg, post-surgical mortality reached 44%.

Quality of biopsy sampling practices for Barrett’s esophagus (BE): Results of a large BE register indicate that endoscopists systematically undersample BE patients in nearly 20% of cases. As BE length increases, endoscopists become less compliant with the Seattle protocol and the odds of non-adherence increases by 31% with every 1-cm increase in length.
