Text:

Herfarth H et al, Gastroenterology. 2018;155(4):1098–108.e9

Methotrexate as maintenance therapy for ulcerative colitis: Parenteral methotrexate (25 mg/week) was not superior to placebo in preventing relapses of ulcerative colitis in patients who achieved steroid-free response during induction therapy.

Bugajski M et al, Gut. 2018;67(11):1958–64

Modifiable factors associated with patient-reported pain during and after screening colonoscopy: Training of the endoscopist, sedation with propofol and adequate bowel preparation are associated with reduced pain intensities.

Kyaw MH et al, Gastroenterology. 2018;155(4):1090–7.e1

Medical therapy of small bowel ulcers in aspirin users with small bowel bleeding: Misoprostol was superior to placebo in promoting healing of small bowel ulcerations/erosions and increasing hemoglobin concentrations in a small randomized trial in patients requiring continuous aspirin therapy.

Liver Biliary Tracts

Text:

Ioannou GN et al, J Hepatol. 2018;69(5):1088–98

Hepatocellular carcinoma (HCC) after hepatitis C treatment: A current study has developed models to predict the HCC risk after antiviral treatment for hepatitis C. These models are available as web-based tools and can help to establish risk-based HCC surveillance for individual patients.

Non-alcoholic fatty liver disease (NAFLD): Operable and cost-efficient management of the NAFLD “epidemic” remains an issue under debate. A current study demonstrates that web-based interventions are not less effective than common lifestyle programs for the treatment of NAFLD.

Non-alcoholic fatty liver disease (NAFLD): Another issue under debate in the management of NAFLD is the optimal dietary regimen. A current study demonstrates that both, low fat and Mediterranean diets, lead to a reduction in hepatic steatosis. Regarding cardiometabolic risk factors and adherence, however, a Mediterranean diet seems to be more favorable.

Oesophagus Stomach Duodenum

Text:

Krag M et al, N Engl J Med. 2018 [in press]

Prophylaxis for gastrointestinal stress ulceration in the intensive care unit: Results of a large phase III trial indicate that prophylaxis with pantoprazole in patients at risk for gastrointestinal bleeding does not affect mortality at 90 days or other clinically important events as compared to placebo. Based on these data, administration of a general prophylaxis with pantoprazole is not justified.

Immune therapy for refractory esophagogastric cancer: Nivolumab or nivolumab plus ipilimumab demonstrated clinically meaningful antitumor activity, durable responses and encouraging long-term overall survival after failure to second-line chemotherapy.

Ribolsi M et al, Aliment Pharmacol Ther. 2018;48(10):1074–81

Prevalence and clinical characteristics of refractoriness to optimal proton-pump inhibitor (PPI) therapy in non-erosive reflux disease (NERD): True refractoriness accounts to approx. 20% of patients. True NERD constitutes only a third of the PPI-refractory group while 42% and 26% rather suffer from reflux hypersensitivity or functional heartburn, respectively.

Pancreas

Text:

Trikudanathan G et al, Am J Gastroenterol. 2018;113(10):1550–8

Timing of endoscopic interventions for necrotizing pancreatitis: Early (< 4 weeks) interventions were performed more often for disease courses with infection and organ failure but were not associated with increase in complications, and resulted in similar improvement in organ failure and relatively low mortality.
