Significance of immunomodulator monotherapy for inflammatory bowel diseases (IBD): Results of a large Canadian cohort study reveal that almost half of IBD patients are treated with a monotherapy of immunomodulators. Approx. one third of these patients achieve a sustained clinical response for over 5 years.

Effectiveness of colorectal cancer (CRC) screening in detecting earlier-stage disease: Nationwide cohort study in Denmark demonstrates that inviting individuals to undergo fecal immunochemical blood test-based screening leads to detection of almost 2-fold more CRC cases than not inviting indviduals.

Novel therapeutic approach to treat functional constipation: A phase III trial indicates that elobixibat, an orally available inhibitor of ileal bile acid transport, resolved constipation in the short-term and is well tolerated in the long-term.

Hepatitis C: Two current studies from Europe and the US, respectively, clearly demonstrate that direct-acting antiviral therapy for hepatitis C led to a decrease in hepatitis C-related liver transplantation, an improved survival after hepatitis C-related liver transplantation, and a decrease in hepatitis C-related mortality. Liver transplantation and mortality due to alcohol and non-alcoholic fatty liver disease, however, are increasing.

Autoimmune hepatitis: A current cohort study confirms a substantial impact of autoimmune hepatitis on health-related quality of life. In this context, the use of corticosteroids has a prominent role.

Liver transplantation: The Model of End-stage Liver Disease (MELD) score is an accepted indicator for the urgency of liver transplantation. In contrast, well-established predictors for the chances of success of liver transplantation are missing. A current study indicates that the Karnofsky performance status may be a feasible predictor of graft and patient survival after liver transplantation.

Natural course and consequences of eosinophilic esophagitis (EoE): A large retrospective cohort study confirms that EoE progresses from an inflammatory to a fibrostenotic phenotype. With each additional year of undiagnosed EoE the risk of stricture presence increases with 9%.

Chemoprevention for Barrett’s esophagus: High-dose proton-pump inhibitors (PPI) and aspirin, especially in combination, significantly and safely improved outcomes in patients with Barrett’s esophagus (mortality, esophageal adenocarcinoma or high-grade dysplasia). The number needed to treat was 34 for PPI and 43 for aspirin.

Second-line chemotherapy for advanced gastric or gastroesophageal junction cancer: The PD-1 inhibitor pembrolizumab did not significantly improve overall survival compared with paclitaxel in a phase III trial, but had a better safety profile.

Risk of neoplastic progression in individuals at high risk for pancreatic cancer: Pancreatic cancer and high-risk precursors in patients undergoing long-term surveillance in 7% within 5 years with a neoplastic progression rate of 1,6% per year. Most tumors detected during surveillance are resectable and the overall survival is good.
