Latest research in brief:

Bowel

Text:

van Dijk ST et al, Am J Gastroenterol. 2018;113(7):1045–52

Antibiotic therapy for uncomplicated acute diverticulitis: Long-term results of a clinical trial indicate that omitting antibiotics does not result in more complicated or recurrent diverticulitis or in increased rates of sigma resections at long-term follow up after 24 months.

Singh S et al, Am J Gastroenterol. 2018;113(8):1197–1205

Combination therapy of 5-aminosalicylates and TNF inhibitors for ulcerative colitis: Pooled analysis of individual participant data from clinical trials indicates that concomitant 5-ASA therapy in patients with moderate-to-severe ulcerative colitis who are escalated to TNF-inhibitors does not improve clinical outcomes.

Kirchgesner J et al, Gastroenterology. 2018;155(2):337–46.e10

Infections in patients with inflammatory bowel disease: A large trial indicates that combination therapy with thiopurines and TNF inhibitors is associated with an increased risk for severe and opportunistic infections. As compared to TNF inhibitors, patients receiving thiopurines carry a reduced risk for severe, bacterial or mycobacterial, but higher risk for viral infections.

Adams D et al, N Engl J Med. 2018;379(1):11–21

Hereditary liver disease: Hereditary transthyretin amyloidosis is a systemic disease and is caused by misfolded transthyretin that is produced in the liver. Liver transplantation is the only therapy to date. Two current studies, however, demonstrate the efficacy of an RNA interference therapy using antisense oligonucleotides.

Primary biliary cholangitis (PBC): A new score based on simple clinical and biochemical parameters may allow accurate prediction of treatment response to ursodeoxycholic acid.

Otete H et al, J Hepatol. 2018;69(3):697–704

Alcoholic cirrhosis: Patients with alcoholic cirrhosis have a markedly increased risk of hip fractures and post-hip fracture mortality compared to the general population. Young patients are at special risk. Thus, fracture prevention is highly needed in this patient group.

Oesophagus Stomach Duodenum

Text:

Schmidt A et al, Gastroenterology. 2018;155(3):674–86.e6

Over-the-scope clips (OTSC) for recurrent peptic ulcer bleeding: Randomized trial confirms that OTSC are more effective than endoscopic standard therapy.

Nagata N et al, Gut. 2018;67(10):1805–12

Therapeutic endoscopy-related GI bleeding and thromboembolic events in patients taking oral anticoagulants: The risk of postendoscopy bleeding was higher in patients taking warfarin as compared to direct oral anticoagulants. Perinterventional bridging with heparin was associated with an increased risk of bleeding, but did not prevent thromboembolism.

Cotton CC et al, Gastroenterology. 2018;155(2):316–26.e6

Surveillance intervals after radiofrequency ablation of Barrett’s esophagus: Analysis of two independent patient registries revealed that intervals for surveillance endoscopies after eradication of Barrett’s esophagus with low-grade dysplasia should be 1 and 3 years, while patients with high-grade dysplasia or intramucosal adenocarcinoma should be followed up after 0,25, 0,5 and 1 year and annually thereafter.

Pancreas

Text:

Bang JY et al, Gut. 2018 [in press]

Endoscopic therapy of pancreatic walled-off necrosis: AExcept for procedure duration, there was no significant difference in treatment outcomes in a trial comparing lumen-apposing metal stents (LAMS) and plastic stents. In contrast, LAMS should be removed within 3 weeks since they may cause significant stent-related complications.
