Bowel

Text:

Freedman SB et al, N Engl J Med. 2018;379(21):2015–26

Combination probiotic therapy in children with gastroenteritis: Administration of a combination probiotic product containing Lactobacillus to children who present with gastroenteritis to the emergency department does not prevent the development of moderate-to-severe gastroenteritis within the following 14 days.

Halkjæer SI et al, Gut. 2018;67(12):2107–15

Feacal microbiota transfer (FMT) in patients with irritable bowel syndrome: Results of a placebo-controlled randomized trial reveal that FMT altered gut microbiota diversity. However, patients in the placebo group experienced greater symptom relief compared with the FMT group after 3 months.

Continuous anti-TNF use throughout pregnancy: A large French retrospective cohort study revealed that anti-TNF treatment during pregnancy increases the risk of maternal complications approximately 1.5-fold, but does not increase the infection risk for exposed children during their first year of life.

Liver Biliary Tracts

Text:

Sarrazin C et al, J Hepatol. 2018;69(6):1221–30

Hepatitis C: A current study demonstrates that resistance-associated substitutions have no impact on the efficacy of sofosbuvir, velpatasvir, and voxilaprevir for 12 weeks in patients with previous failure of a direct-acting antiviral treatment. Even a previous therapy with an NS5A inhibitor had no negative effect.

Non-alcoholic fatty liver disease (NAFLD): Therapeutic options in non-obese patients with NAFLD are so far not well defined. A current study demonstrates that lifestyle interventions are effective in this patient group, too. In contrast to obese NAFLD patients, a moderate weight reduction of approx. 5% may be sufficient in this patient group.

Decompensated liver cirrhosis: The recently published ANSWER trial (Caraceni et al. Lancet. 2018;391[10138]:2417–29) demonstrated a prognostic benefit of a long-term substitution of human albumin in patients with decompensated liver cirrhosis. This controversially discussed finding could not be confirmed by a similar current trial. Indeed, long-term treatment with albumin and the α-adrenergic midodrine had no effect on complications or survival.

Endoscopic therapy for refractory gastroparesis: Multicenter observational study suggests that gastric peroral endoscopic myotomy (G-POEM) may be technically feasible, safe and successful for the treatment of refractory gastroparesis.
