Castells A et al, Lancet. 2025;405(10486):1231-1239

Colorectal cancer screening by invitations to fecal immunochemical testing (FIT) or colonoscopy: In a large Spanish randomized trial, participation in screening was higher among individuals invited to FIT screening than colonoscopy screening. On the basis of participation observed in this study, a FIT-based program was non-inferior to a colonoscopy-based program in terms of colorectal cancer-related mortality after 10 years.

Endoscopic colorectal cancer screening in inflammatory bowel disease (IBD) – time matters: In the randomized HELIOS trial, high-definition white-light endoscopy with re-inspection of all bowel segments was not inferior to dye-based chromoendoscopy in the detection of neoplastic lesions. Advantages of chromo- over conventional endoscopy could therefore be explained by longer inspection times rather than on dye-related advantages.

Lindvig KP et al, Lancet Gastroenterol Hepatol. 2025;10(1):55-67

The next step in the non-invasive diagnosis of significant liver fibrosis: Steatotic liver disease (SLD) is common, affecting up to 30% of the population. Non-invasive screening tests could help to identify individuals with a progressive form of SLD with the development of fibrosis/cirrhosis. In this study of nearly 7000 individuals from Denmark, Germany, and the United Kingdom, a new score was developed that can predict the presence of significant liver fibrosis with high accuracy based on age and 3 to 9 blood test results.

Improvement of liver grafts through hypothermic oxygenated perfusion (HOPE) before transplantation: In this international study from 10 transplant centers, the perfusates of 473 liver grafts treated with HOPE before transplantation were examined. The detection of defined mitochondrial compounds in the perfusate was associated with graft loss, cholangiopathy, and renal failure after liver transplantation. Overall, HOPE can increase the quality of marginal organs, and the findings from this study could improve the assessment of graft quality.

Tessier AJ et al, Nat Med. 2025 [ahead of print]

Optimal diets for healthy aging: Results of 2 large longitudinal cohort studies confirm that higher intakes of fruits, vegetables, whole grains, unsaturated fats, nuts, legumes and low-fat dairy products were linked to greater odds of healthy aging, whereas higher intakes of trans fats, sodium, sugary beverages and red or processed meats (or both) were inversely associated.

Adler J et al, Gut. 2025;74(4):539-546

Early TNF antibody therapy protects against disease progression and perianal fistulas in Crohn's disease: Re-analysis of the pediatric RISK inception cohort demonstrates that the presence of perianal skin tags, fissures or ulcerations is associated with a high risk of developing perianal fistulas. Early TNF antibody therapy reduced the risk of perianal fistulas in the RISK cohort by 84% and, in the presence of perianal risk stigmata, by as much as 94%.

Aspirin and pancreatic cancer risk in diabetes: In a very large retrospective cohort study of patients with diabetes from Hong Kong, treatment with aspirin was associated with a lower risk of pancreatic cancer and lower pancreatic cancer-associated mortality.
