Calcium and vitamin D supplementation and risk of serrated polyps: A placebo-controlled chemoprevention trial reveals that supplementation with calcium or calcium/vitamin D is associated with a 2.7- and 3.8-fold increased risk for the development of sessile serrated adenoma and polyps. Increased risk was a late effect observed 6–10 years after initiation of supplementation.

Appendectomy for therapy-refractory ulcerative colitis: Results of a pilot study involving 30 patients suggest that appendectomy may be effective in 30% of patients, which correlated with active inflammation within the appendix, suggesting that a subgroup of ulcerative colitis patients may benefit substantially from appendectomy.

Hepatitis D: The current standard of care for chronic HDV/HBV infection is a 48-week course of pegylated interferon-α, leading to HDV clearance in 25–30% of patients only. The recently published HIDIT-II study demonstrates that neither a treatment prolongation to 96 weeks nor the combination with tenofovir is of substantial benefit.

Hepatitis C: A current prospective cohort study from France including 9895 patients (3/4 treated during the follow-up, 1/4 untreated) demonstrates that treatment with direct-acting antivirals (DAA) is associated with a markedly reduced risk of mortality and hepatocellular carcinoma. The risk for decompensated cirrhosis, however, was not affected by DAA treatment in this cohort.

Hepatitis C: A current global mathematical model reveals that further improvements in blood safety and infection control, expansion of creation of harm reduction services for people who inject drugs, and extensive screening for hepatitis C virus (HCV) with concomitant treatment for all are necessary to reduce the burden of HCV. Only a consequent implementation of these measures will allow to reach the WHO aims (HCV incidence reduction of 80% and mortality reduction of 65% by 2030).

Screening for gastric cancer in low incidence regions: A prospective trial from the Netherlands and Norway indicates that patients with atrophic gastritis, intestinal metaplasia or dysplasia have a neoplastic progression rate of 0.3% per year. Endoscopic surveillance programs can detect cancer at an early and curable stage.

ening of asymptomatic individuals at high-risk for pancreatic cancer: Meta-analysis reveals that 135 patients must be screened by endosonography or magnetic resonance imaging to identify 1 patient with a high-risk pancreatic lesion. These findings raise the question whether such screening approaches are reasonable and cost-effective.
