Bowel

Text:

Ponsioen CY et al, Lancet Gastroenterol Hepatol. 2017;2(11):785–92

Therapy of non-stricturing terminal ileitis in Crohn’s disease after failure of conventional therapy: Laparoscopic ileocoecal resection and infliximab are equally effective with regard to patient-reported outcomes.

Text:

Olén O et al, BMJ. 2017;358:j3951

Cancer risk in inflammatory bowel disease (IBD): Patients with childhood onset of IBD have an increased risk of cancer, especially gastrointestinal cancers, both in childhood and later in life. This elevated risk has been relatively stable over the last 50 years.

Chang SH et al, JAMA. 2017;318(13):1250–9

Risk of major bleeding in non-valvular atrial fibrillation: Concomitant therapy of non-vitamin K oral anticoagulants with amiodarone, fluconazole, rifampin or phenytoin results in increased risk of major bleeding.

Liver Biliary Tracts

Text:

Papatheodoridis GV et al, Hepatology. 2017;66(5):1444–53

Chronic hepatitis B and hepatocellular carcinoma (HCC): The HCC risk decreases beyond year 5 of entecavir or tenofovir therapy, particularly in patients with cirrhosis. Older age (especially ≥ 50 years), lower platelets, and liver stiffness ≥ 12 kPa at year 5 are main risk factors for late HCC development.

Text:

Nagata H et al, J Hepatol. 2017;67(5):933–9

Chronic hepatitis C and hepatocellular carcinoma (HCC): The risk of early HCC occurrence or recurrence after viral eradication is similar between interferon-based and interferon-free therapies. Post-treatment levels of Wisteria floribunda agglutinin-positive Mac-2 binding protein (WFA+M2BP) may be a helpful screening biomarker for the risk of HCC after interferon-free therapy, even in the absence of severe fibrosis.

Text:

Taylor EJ et al, Hepatology. 2017;66(5):1546–55

Surveillance for hepatocellular carcinoma (HCC): There is a balance between the small absolute mortality benefit and the numerically more frequent harms from false-positive testing. In a current simulation study, 1 out of 7 patients experienced a false-positive testing, resulting in additional computer tomography scans, magnetic resonance imaging and liver biopsies.

Oesophagus Stomach Duodenum

Text:

Greuter T et al, Am J Gastroenterol. 2017;112(10):1527–35

Therapy of eosinophilic esophagitis: Long-term treatment with swallowed topical corticosteroids is well tolerated, however, only a minority of patients achieves clinical, endoscopic and histological remission (deep remission). A large majority of these patients relapses upon discontinuation of therapy.in triple therapy in the second-line treatment.

Lochhead P et al, Gastroenterology. 2017;153(4):971–9.e4

Proton-pump inhibitor (PPI) therapy and risk of dementia: Data from the Nurses’ Health Study II do not show a convincing association between long-term PPI use and cognitive function. Therefore, these data do not support the suggestion that PPI use increases dementia risk.

GTherapy of diabetic gastroparesis: Results of a phase 2b trial indicate that the ghrelin agonist relamorelin significantly improves core symptoms of gastroparesis and accelerates gastric emptying.
